We work in partnership with local authorities to re-able individuals who have recently been discharged from a hospital stay. We have a dedicated team of care workers who are committed to promoting independence and supporting your loved one to return to their normal routine and lifestyle as soon as possible.

NHS Continuing Care, also known as Intermediate Care, is a free service provided by the NHS to individuals who have significant health needs. It can be received in any setting outside of a hospital, including your own home.

Your service and care so far have been excellent. The staff have been very friendly and efficient. My request to have female carers has been a great help to me.

A person’s needs, not their location or income, is the key factor in receiving this help.

Wisteria Care are ‘trusted assessors’ meaning the local authority trust us to carry out an assessment on whether care is needed for the long term, or if it is only a temporary solution to support your loved one in regaining their confidence in their own abilities. This assessment period is up to six weeks, after which time we will determine what support, if any, is needed in the future to enable your loved one to stay in their own home, with the appropriate care package in place.
